UNEX-42 (unexisome) is an exosome-based therapeutic candidate consisting of extracellular vesicles (EVs) derived from human bone marrow mesenchymal stem cells (BMSCs). Developed by United Therapeutics, it is currently being investigated in clinical trials for the prevention and treatment of bronchopulmonary dysplasia (BPD) in extremely preterm neonates. The therapy functions by delivering a complex cargo of bioactive molecules, including proteins, lipids, and various RNA species, which exert paracrine effects to modulate inflammatory responses, reduce fibrosis, and promote alveolar and vascular growth in the developing lung. By leveraging the regenerative properties of MSC-derived vesicles without the risks associated with whole-cell transplantation, UNEX-42 aims to mitigate the lung injury caused by mechanical ventilation and oxygen toxicity in high-risk infants.
