A combination therapy using both unfractionated heparin (UFH) and low-molecular-weight heparin (LMWH) simultaneously or sequentially. This approach leverages the complementary mechanisms of these anticoagulants, with UFH primarily inhibiting thrombin (factor IIa) and LMWH having greater activity against factor Xa. The combination has demonstrated synergistic effects in enhancing anti-factor Xa activity and releasing tissue factor pathway inhibitor, providing more effective prevention of venous thrombosis than either agent alone in experimental models.
