Unithiol (sodium 2,3-dimercapto-1-propanesulfonate, DMPS) is a water-soluble dithiol chelating agent developed as an analogue of dimercaprol (BAL). It contains two vicinal sulfhydryl groups that exhibit high affinity for heavy metal cations, including mercury, arsenic, lead, and copper. By forming stable, non-toxic, water-soluble complexes (mercaptides), unithiol facilitates the mobilization of metals from tissues and their subsequent renal excretion. It is primarily indicated for the treatment of acute and chronic heavy metal poisoning and is being investigated for its efficacy in treating peripheral neuropathy associated with occupational metal exposure. It is widely used in clinical practice in China and Eastern Europe for occupational toxicology.
