Overview
**Unmanipulated cord blood unit** refers to a stem cell product derived from umbilical cord blood that has been processed only to the extent necessary for cryopreservation, banking, and clinical transplantation, without ex vivo expansion or genetic modification[3][4]. The units contain hematopoietic stem and progenitor cells and other cell populations capable of *hematopoietic reconstitution* and immune system regeneration[6]. The mechanism of action is primarily engraftment of donor hematopoietic stem cells in patients with inherited, acquired, or treatment-induced hematopoietic disorders, leading to restoration of healthy blood and immune system function[3][6]. Unmanipulated cord blood is "antigenically naïve," which can reduce graft-versus-host disease rates versus other sources, and is used as an alternative when HLA-matched adult donors are unavailable[1][6].
