Unmanipulated umbilical cord blood is a biological cell therapy consisting of hematopoietic stem and progenitor cells (HSPCs) collected from the umbilical cord and placenta after childbirth. Unlike ex vivo expanded or engineered cord blood products, this intervention is infused without prior manipulation or expansion. It is primarily used in allogeneic hematopoietic stem cell transplantation (HSCT) to treat hematologic malignancies, such as acute myeloid leukemia (AML) and lymphoma, as well as various genetic disorders. The mechanism of action involves the engraftment of donor stem cells into the recipient's bone marrow following conditioning regimens, leading to hematopoietic reconstitution and the establishment of a new immune system. This process can also provide a graft-versus-tumor effect, where the donor-derived immune cells help eliminate residual malignant cells.
