Ustekinumab + vedolizumab is a combination of two monoclonal antibody biologics used as dual therapy, primarily in the management of refractory Crohn’s disease (CD) and other forms of inflammatory bowel disease (IBD). Ustekinumab is a fully human monoclonal antibody that targets the p40 subunit shared by interleukin-12 (IL-12) and interleukin-23 (IL-23), thereby inhibiting their pro-inflammatory signaling. Vedolizumab is a humanized monoclonal antibody that specifically binds to the α4β7 integrin on lymphocytes, blocking its interaction with mucosal addressin cell adhesion molecule 1 (MAdCAM-1), which inhibits leukocyte migration into gastrointestinal tissue. The rationale for combining these agents lies in their complementary mechanisms—ustekinumab modulates systemic immune responses while vedolizumab restricts gut-specific lymphocyte trafficking—offering potential benefit for patients with IBD who are refractory to monotherapy or have extraintestinal manifestations. Early clinical evidence suggests this combination can induce remission in difficult-to-treat cases, though large-scale studies are still needed to confirm efficacy and safety[2][3][5][9].
