Ustekinumab and Vancomycin Combination Therapy

The combination of ustekinumab and vancomycin has been documented in several case reports as an effective treatment approach for refractory ulcerative colitis, particularly in patients with concurrent primary sclerosing cholangitis (PSC). ## Therapeutic Use and Effectiveness This combination therapy has shown promising results in treating ulcerative colitis that has not responded to standard treatments. Notably, when attempts were made to discontinue vancomycin while maintaining ustekinumab therapy, the patient experienced symptom recurrence and increased inflammatory markers that improved upon restarting vancomycin. Another case involved a pediatric patient with primary sclerosing cholangitis who showed significant improvement with this combination therapy. After 18 days of treatment with ustekinumab plus vancomycin, the patient's condition improved dramatically, with bowel movements decreasing from 8 bloody loose movements per day to 2 solid bowel movements per day. The patient maintained clinical improvement with normalized laboratory parameters after the introduction of vancomycin. ## Mechanism and Rationale While antibiotics are generally not standard treatment for ulcerative colitis, vancomycin appears to provide additional benefit when combined with biologic therapy in certain cases. Vancomycin is a glycopeptide antibiotic that is poorly absorbed in the gastrointestinal tract. The mechanism behind vancomycin's effectiveness in this combination may involve: 1. Immunomodulatory effects on T lymphocytes 2. Antimicrobial activity against unknown gastrointestinal pathogens or effects on the normal microbiome 3.
