**UThymoKidney** is an investigational xenotransplantation product developed by United Therapeutics for **end-stage renal disease**. It consists of a kidney from a GalSafe donor pig carrying a single **GGTA1** gene knockout, transplanted together with thymus tissue from the same donor pig. Loss of alpha-1,3-galactosyltransferase prevents expression of the alpha-gal carbohydrate antigen, reducing a major source of immediate xenograft rejection; the accompanying thymic tissue is intended to promote recipient immune recognition of the graft as self and reduce rejection risk. The EXTEND program is evaluating its safety and efficacy in patients with ESRD.
