Utomilumab is a fully human IgG2 monoclonal antibody that acts as an agonist of the 4-1BB (CD137) receptor, a costimulatory protein expressed on activated T cells and natural killer (NK) cells. By binding to 4-1BB, utomilumab stimulates and increases the number and activity of immune effector cells, potentially enhancing anti-tumor immune responses. It is being developed primarily for cancer immunotherapy, both as a single agent and in combination with other therapies such as rituximab or checkpoint inhibitors like pembrolizumab. Utomilumab has demonstrated a favorable safety profile in early-phase clinical trials but has shown limited single-agent efficacy in patients with advanced solid tumors or those refractory to immune checkpoint inhibitors[3][5][8]. The drug is under investigation for several malignancies including breast cancer, oropharyngeal cancer, diffuse large B cell lymphoma, solid tumors, squamous cell cancer, follicular lymphoma, ovarian cancer, and non-Hodgkin's lymphoma[4].
