Vaginal microbiota transplant (VMT) is an investigational biological therapy being developed by Douglas Kwon and Caroline Mitchell for the treatment of recurrent bacterial vaginosis (BV). The therapy involves the transfer of the entire vaginal microbial community from healthy, Lactobacillus-dominant donors into patients with BV following a course of antibiotic treatment (typically metronidazole). The goal of the transplant is to achieve stable engraftment of beneficial bacteria, particularly *Lactobacillus crispatus*, to restore a healthy vaginal ecosystem, reduce inflammation, and prevent the recurrence of dysbiosis. It is currently being evaluated in the Phase 1/2 MOTIF trial (NCT04046900) and is regulated by the FDA as a live biotherapeutic product under Investigational New Drug (IND) status.
