This is a combination therapy of two antiepileptic drugs, valproate (also known as valproic acid or valproate sodium) and carbamazepine. Both are small molecule anticonvulsants used primarily in the management of epilepsy and certain mood disorders. Valproate increases gamma-aminobutyric acid (GABA) levels in the brain by inhibiting GABA transaminase and also affects voltage-gated sodium channels. Carbamazepine acts mainly by blocking voltage-gated sodium channels, stabilizing hyperexcited nerve membranes, inhibiting repetitive neuronal firing, and reducing synaptic propagation of excitatory impulses. The combination may be considered when monotherapy is insufficient for seizure control; however, it carries significant risks due to complex pharmacokinetic interactions that can alter serum concentrations of both drugs and increase toxicity risk[1][4]. This combination should only be used with careful monitoring.
