Valproic acid + dexamethasone is a combination regimen investigated by the Barbara Ann Karmanos Cancer Institute for the induction of folate receptor alpha (FRα) as a diagnostic and surveillance biomarker. Valproic acid, a small molecule histone deacetylase (HDAC) inhibitor, and dexamethasone, a synthetic glucocorticoid receptor agonist, are used together to synergistically upregulate the expression of FRα in ovarian cancer cells. This induction is intended to increase the shedding of the receptor into the serum, allowing for more sensitive detection and monitoring of ovarian carcinoma and adnexal masses through laboratory biomarker analysis. The approach is being evaluated in a Phase 1 pilot study to determine if this pharmacological induction can improve the clinical utility of serum folate receptor levels in diagnosing and tracking disease recurrence.
