Variant-adapted BNT162b2 refers to a family of updated formulations of the Pfizer-BioNTech mRNA COVID-19 vaccine Comirnaty, in which the encoded SARS-CoV-2 spike glycoprotein sequence is modified to match specific variants of concern (e.g., Omicron BA.1, BA.4/BA.5, XBB.1.5, JN.1, KP.2) while retaining the original BNT162b2 lipid nanoparticle–mRNA platform. These vaccines deliver nucleoside-modified mRNA encoding a prefusion-stabilized full-length spike protein from the targeted lineage; host cells translate this mRNA and express spike on their surface, eliciting neutralizing antibodies and spike-specific CD4+ and CD8+ T-cell responses that improve protection against infection, symptomatic disease, hospitalization, and death from contemporary SARS-CoV-2 variants compared to the original ancestral-strain vaccine.[1][2][3][4] Early formulations were bivalent, combining ancestral and Omicron spike sequences to broaden cross-neutralization, whereas more recent monovalent XBB.1.5-, JN.1-, and KP.2-adapted versions have been developed to address major antigenic shifts in circulating lineages, with immunogenicity-bridging data showing substantially higher neutralizing titers against matched and related sublineages than prior formulations.[1][2][4] Variant-adapted BNT162b2 vaccines are developed and commercialized jointly by BioNTech and Pfizer and are primarily indicated as booster and, in some settings, primary vaccination against COVID-19 in alignment with periodically updated regulatory strain-selection guidance.[1][2][3][6]
