Overview

This is a four-agent combination immunotherapy regimen under investigation for the treatment of advanced melanoma and other cancers. The components are: **Varlilumab** is a fully human monoclonal antibody that acts as an agonist to CD27, a co-stimulatory molecule on T cells, enhancing T-cell activation and anti-tumor immune responses[1][2][7]. **Ipilimumab (Yervoy)** is a human monoclonal antibody that blocks CTLA‑4, an immune checkpoint receptor, thereby promoting T-cell activation and proliferation against tumor cells[1]. **CDX‑1401** is an antibody fusion protein designed to deliver the NY‑ESO‑1 tumor antigen directly to dendritic cells by targeting DEC‑205; this facilitates efficient antigen presentation and robust anti-tumor immunity in patients whose tumors express NY‑ESO‑1[6][8]. **Poly‑ICLC (Hiltonol)** is a synthetic double-stranded RNA acting as an adjuvant; it activates innate immunity via Toll-like receptor 3 (TLR3) and other cytoplasmic receptors, inducing type I interferon production and proinflammatory cytokines to further enhance vaccine-induced immune responses[3][6]. The combination aims to maximize anti-tumor immunity by simultaneously activating multiple arms of the immune system—T cell co-stimulation (varlilumab), checkpoint inhibition (ipilimumab), targeted antigen delivery/vaccination (CDX–1401), and innate immune stimulation/adjuvancy (poly–ICLC). This regimen has been studied in phase 1/2 clinical trials for unresectable stage III or IV melanoma expressing NY–ESO–1[1][5].
