Overview

VB-111 + nivolumab is an investigational combination therapy for cancer. It consists of: - **VB-111 (ofranergene obadenovec):** a genetically modified adenoviral vector gene therapy designed to target the tumor microenvironment by inducing apoptosis in angiogenic blood vessels and stimulating an immune response. Its dual mechanism includes antiangiogenic effects (disrupting neovascularization) and immunomodulation. - **Nivolumab:** a monoclonal antibody checkpoint inhibitor targeting PD-1 on T cells, thereby blocking the interaction with PD-L1/PD-L2 on tumor cells and restoring T cell-mediated antitumor immunity. This combination has been studied primarily in patients with microsatellite stable colorectal liver metastases (MSS CLM), aiming to convert immunologically "cold" tumors into "hot" ones that are more responsive to immunotherapy. In phase II trials, the regimen was tolerated with minimal toxicities but did not improve overall response rate or survival compared to historical controls[1][5][3]. The rationale for combining these agents is that VB-111 may enhance immune infiltration into tumors, potentially synergizing with checkpoint inhibition[6][7].
