VB-201

VB-201 is a small molecule, orally available, oxidized phospholipid analog (lecinoxoid) developed as an anti-inflammatory and immunomodulatory agent. It acts primarily by inhibiting innate immune activation through antagonism of Toll-like receptor 2 (TLR-2), Toll-like receptor 4 (TLR-4), and CD14 antigen, thereby reducing monocyte and dendritic cell activation and migration. This mechanism leads to decreased inflammation in diseases such as atherosclerosis, psoriasis, inflammatory bowel disease (IBD), rheumatoid arthritis, multiple sclerosis, and COVID-19 infections. VB-201 was originally developed by VBL Therapeutics (now Notable Labs). It has been investigated in clinical trials up to Phase II for several indications including psoriasis, atherosclerosis, COVID-19 infection, arteritis, HIV-related inflammation, ulcerative colitis among others. Despite some promising preclinical results showing reduction of vascular inflammation and immune modulation without affecting cholesterol levels or neutrophil function, development was discontinued for plaque psoriasis and atherosclerosis due to lack of significant efficacy in Phase II trials. The drug continues development for other inflammatory conditions with ongoing clinical studies.[1][2][5][6][7][9]
