VB6-845d is a targeted protein therapeutic designed as a systemically administered antibody–drug conjugate for the treatment of EpCAM-positive solid tumors. It comprises a humanized anti-EpCAM Fab fragment genetically fused to deBouganin (a de-immunized plant toxin derived from bouganin) via a furin-sensitive linker. Upon binding to EpCAM on tumor cells, VB6-845d is internalized and deBouganin inactivates ribosomes, resulting in inhibition of protein synthesis and cell death. Preclinical studies show that VB6-845d induces immunogenic cell death (ICD) with hallmark features such as calreticulin exposure, ATP secretion, and HMGB1 release, suggesting a potential synergy with immune checkpoint inhibitors. VB6-845d aims to overcome multidrug resistance and has been evaluated for systemic use in EpCAM-expressing cancers[2][4][5][10][12][15][16].
