Overview

Vedolizumab is a recombinant humanized IgG1 monoclonal antibody that acts as a selective integrin receptor antagonist. It specifically binds to the α4β7 integrin on lymphocytes, blocking its interaction with mucosal addressin cell adhesion molecule-1 (MAdCAM-1) on gut endothelial cells. This mechanism prevents the migration of inflammatory white blood cells into gastrointestinal tissue, thereby reducing inflammation associated with moderate to severe ulcerative colitis and Crohn's disease. Vedolizumab is gut-selective and does not cause systemic immunosuppression, distinguishing it from other biologics used in inflammatory bowel disease. It was developed for patients who have not responded adequately to conventional therapies.
