vemurafenib + cytarabine + cladribine is a combination therapy regimen being investigated by the Federal Research Institute of Pediatric Hematology, Oncology and Immunology for the treatment of Langerhans cell histiocytosis (LCH). The regimen combines the BRAF inhibitor vemurafenib with two antimetabolite chemotherapeutic agents, cytarabine and cladribine (also known as 2-chlorodeoxyadenosine). This combination is specifically targeted at pediatric patients with multisystem LCH harboring the BRAF V600E mutation. The rationale for this triple therapy is to combine the rapid clinical response provided by targeted BRAF inhibition with the cytotoxic effects of low-dose chemotherapy to achieve a durable complete response and prevent the disease reactivation often observed upon discontinuation of BRAF inhibitor monotherapy.
