This is a combination regimen consisting of venetoclax, azacitidine, and a modified BUCY conditioning protocol. Venetoclax is an oral small molecule inhibitor of B-cell lymphoma 2 (BCL-2), which promotes apoptosis in malignant cells by antagonizing the anti-apoptotic effects of BCL-2. Azacitidine is a hypomethylating agent that incorporates into DNA and RNA, leading to DNA hypomethylation and direct cytotoxicity to abnormal hematopoietic cells. The Modified BUCY regimen typically refers to an adjusted version of the traditional busulfan plus cyclophosphamide (BuCy) conditioning used prior to stem cell transplantation; modifications may include additional agents such as semustine or cytarabine or altered dosing schedules for improved tolerability. This triplet regimen may be explored in high-risk hematologic malignancies such as acute myeloid leukemia (AML) or lymphoma, particularly in patients undergoing autologous stem cell transplantation or those not eligible for intensive induction therapy[1][6][7].
