Venetoclax is a potent, selective, orally bioavailable small-molecule inhibitor of B-cell lymphoma 2 (BCL-2), an anti-apoptotic protein. In the context of relapsed or refractory T-lymphoblastic lymphoma (T-LBL) and T-lymphoblastic leukemia (T-ALL), it is being investigated by Shanghai General Hospital in combination with the IM2 chemotherapy regimen. The IM2 regimen typically consists of ifosfamide, mitoxantrone (or idarubicin/liposomal mitoxantrone), and methotrexate. BCL-2 is often overexpressed in T-cell malignancies, where it prevents apoptosis of cancer cells; venetoclax binds to the BH3-binding groove of BCL-2, displacing pro-apoptotic proteins and triggering programmed cell death. This combination therapy aims to overcome resistance in aggressive T-cell hematological cancers by combining targeted apoptosis induction with traditional cytotoxic chemotherapy.
