This is a combination drug consisting of three active pharmaceutical ingredients: verapamil, trandolapril, and hydrochlorothiazide. Verapamil is a calcium channel blocker that inhibits L-type calcium channels in vascular smooth muscle and myocardial tissue, leading to vasodilation and reduced heart contractility. Trandolapril is an angiotensin-converting enzyme (ACE) inhibitor that blocks the conversion of angiotensin I to angiotensin II, resulting in vasodilation and decreased aldosterone secretion. Hydrochlorothiazide is a thiazide diuretic that increases the excretion of sodium and water by inhibiting sodium reabsorption in the distal tubules of the kidney. The combination targets multiple mechanisms involved in blood pressure regulation—vasodilation via two distinct pathways (calcium channel blockade and ACE inhibition) as well as reduction of blood volume through diuresis—making it potentially effective for treating hypertension, especially when monotherapy does not achieve adequate control.
