Overview

Very low nicotine tobacco refers to tobacco products, primarily combustible types such as cigarettes, that have been engineered to contain nicotine at levels so low (typically ≤0.4 mg/g tobacco, or ~95% less than conventional cigarettes) that they are minimally addictive or nonaddictive. These products deliver less nicotine, thereby reducing or eliminating the addictive properties of tobacco while maintaining the toxicant exposure associated with tobacco combustion. The mechanism of action is a reduction of nicotine delivery to the central nervous system, lowering reinforcement and dependence. Clinical evidence suggests switching to very low nicotine tobacco reduces cigarette consumption, dependence, and may increase quit attempts, but these products are not safer than conventional tobacco with regard to carcinogen/toxin exposure, as the risks primarily result from combustion products rather than nicotine itself[1][2][5]. No medicinal or approved cessation claims exist; rather, these are under consideration as population-level harm reduction tools or regulatory public health interventions, such as a possible FDA-mandated nicotine cap[3].
