**Vesatolimod + tenofovir disoproxil fumarate** is a combination therapeutic investigated primarily for **chronic hepatitis B** and under study for HIV. Vesatolimod is an oral small molecule **agonist of toll-like receptor 7 (TLR7)** designed to boost host immune responses against viral infections, notably through induction of interferon-stimulated genes, activation of natural killer (NK) cells and T cells, and increased interferon-alpha. Tenofovir disoproxil fumarate is an orally administered **nucleotide reverse transcriptase inhibitor (NRTI)** that directly suppresses viral DNA replication by targeting the hepatitis B virus polymerase and, in HIV, the reverse transcriptase enzyme. This combination was assessed in clinical trials for safety, tolerability, pharmacodynamics, immunologic effects, and viral suppression, with vesatolimod serving primarily as an immune modulator and tenofovir disoproxil fumarate as the direct antiviral. The combination did not result in significant additional HBsAg clearance beyond standard antiviral therapy[1][7]. Vesatolimod was developed by Gilead Sciences and is currently being evaluated under various clinical trial settings for chronic hepatitis B and HIV[1][6][3][2][4].
