Alutard SQ 802 Vespula spp. is a subcutaneous allergen immunotherapy (SCIT) product developed by ALK (ALK-Abelló) for the treatment of IgE-mediated wasp venom allergy. It consists of a standardized extract of venom from multiple *Vespula* species, including *V. germanica*, *V. alascensis*, *V. maculifrons*, *V. flavopilosa*, *V. pensylvanica*, and *V. squamosa*. The allergens are adsorbed onto aluminium hydroxide, which serves as an adjuvant to create a depot effect, allowing for slow release and prolonged immune stimulation. The primary mechanism of action involves inducing immunological tolerance by shifting the immune response from a Th2-mediated allergic profile to a Th1/Treg-mediated regulatory profile, characterized by a decrease in specific IgE levels and an increase in protective IgG4 antibodies. This desensitization process significantly reduces the risk of systemic anaphylactic reactions upon subsequent stings. It is currently used in various clinical protocols, including accelerated outpatient regimens, to achieve maintenance dosing and clinical efficacy more rapidly.
