Vicasinabin (RG7774) is a novel, highly selective, and orally bioavailable small molecule agonist of the cannabinoid receptor type 2 (CB2R). Developed as an oral treatment for diabetic retinopathy (DR), particularly in patients with moderately severe to severe non-proliferative diabetic retinopathy (NPDR) and good vision, vicasinabin was designed to offer a less invasive alternative to current therapies that require intraocular injections. Preclinical studies demonstrated that RG7774 reduced retinal vascular permeability, leukocyte adhesion, and ocular inflammation in rodent models. In clinical trials, it showed favorable systemic and ocular safety profiles but did not meet primary efficacy endpoints for improving DR severity. Development was discontinued after Phase II trials due to lack of significant efficacy[3][6][7][9].
