Overview

This is an oral combination therapy consisting of **vildagliptin**, a dipeptidyl peptidase-4 (DPP-4) inhibitor, and an **alpha-glucosidase inhibitor** (class includes acarbose, voglibose, miglitol) for the treatment of type 2 diabetes mellitus. - **Vildagliptin** inhibits the DPP-4 enzyme, increasing endogenous levels of incretin hormones (GLP-1 and GIP), thereby enhancing glucose-dependent insulin secretion from pancreatic beta cells and suppressing glucagon secretion from alpha cells, resulting in improved glycemic control. - **Alpha-glucosidase inhibitors** delay the digestion and absorption of carbohydrates in the intestine by inhibiting alpha-glucosidase enzymes, which reduces postprandial blood glucose spikes; these agents can also promote endogenous GLP-1 secretion. - The combination targets both fasting and postprandial glucose, providing additive or synergistic improvement in glycemic parameters (FBG, PPG, HbA1c) with minimal risk of weight gain or hypoglycemia. This combination is used in patients who do not achieve glycemic targets with either agent alone[1].
