Vildagliptin + glinides is a combination therapy used for the treatment of type 2 diabetes mellitus. Vildagliptin is a selective dipeptidyl peptidase-4 (DPP-4) inhibitor that works by preventing the degradation of the incretin hormones glucagon-like peptide 1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP, thereby increasing their concentration, promoting glucose-dependent insulin secretion, and suppressing inappropriate glucagon secretion[3]. Glinides are rapid-acting insulin secretagogues (such as repaglinide and nateglinide) that stimulate pancreatic beta cells to release insulin, primarily targeting postprandial blood glucose control by closing ATP-dependent potassium channels. The combination offers a complementary mechanism, with vildagliptin enhancing incretin action and glinides directly stimulating insulin release, resulting in better glycemic control without significant risk of hypoglycemia or weight gain.
