**vildagliptin + pioglitazone** is a fixed-dose oral combination of two small molecule antidiabetic agents used for the management of type 2 diabetes mellitus. Vildagliptin is a selective inhibitor of dipeptidyl peptidase-4 (DPP-4), preventing degradation of incretin hormones (GLP-1 and GIP), thereby enhancing glucose-dependent insulin secretion and reducing glucagon levels. Pioglitazone is an agonist of peroxisome proliferator-activated receptor gamma (PPARγ), a nuclear receptor that modulates the transcription of genes involved in glucose and lipid metabolism, improving insulin sensitivity in peripheral tissues. The combination provides complementary mechanisms to improve glycemic control as first-line or add-on therapy when monotherapy is insufficient. It is mainly indicated for adults with type 2 diabetes and is available in tablet form[1][3][5][6].
