Overview

Maintenance therapy consisting of vinblastine and cyclophosphamide, used as the final phase of the CombinaiR3 multimodal treatment strategy for patients with very high-risk, primary extrapulmonary metastatic Ewing sarcoma. Vinblastine is a vinca alkaloid that binds to tubulin and inhibits microtubule formation, leading to mitotic arrest in the metaphase. Cyclophosphamide is a nitrogen mustard alkylating agent that is metabolized into active forms that create DNA cross-links, thereby inhibiting DNA synthesis and cell division. This specific maintenance regimen was evaluated by Institut Curie and collaborators in the CombinaiR3 Phase 2 trial (NCT03011528) following intensive induction chemotherapy (VDC-IE), response-adapted TEMIRI, and high-dose BuMel consolidation to prevent relapse in patients with disseminated disease.
