Overview

The vinblastine + prednisone combination is a chemotherapy protocol commonly used in veterinary medicine, particularly for treating canine mast cell tumors (MCTs). This combination has been extensively studied and shows good efficacy in both neoadjuvant and adjuvant settings. ## Mechanism of Action Vinblastine is a plant alkaloid that binds to tubulin, disrupting microtubule formation and therefore mitosis[1][4]. It becomes heavily bound to proteins in the blood and does not distribute into the central nervous system. Vinblastine is metabolized in the liver and largely excreted in bile and feces, with some excretion in urine[5]. Prednisone (or prednisolone) is a corticosteroid that works synergistically with vinblastine in this combination therapy. The combination has shown better efficacy than either agent used alone, particularly for mast cell tumors in dogs. ## Clinical Applications The primary use of vinblastine + prednisone is for treating high-grade or metastatic mast cell tumors in dogs. A conventional protocol consists of 8 vinblastine doses in combination with daily oral prednisolone therapy over a 12-week period[3]. Vinblastine is typically administered intravenously at 2 mg/m², once weekly for the first 4 weeks and then every 2 weeks for the final 8 weeks[3][5]. In human medicine, similar combinations have been used for treating Hodgkin's disease, though often with additional agents such as chlorambucil and procarbazine (ChIVPP regimen)[2]. ## Efficacy and Outcomes Studies have shown that prednisone and vinblastine chemotherapy is well-tolerated and results in good outcomes following surgery in dogs with MCTs at high risk for metastasis[7]. One study demonstrated that this combination is an active regimen for the treatment of MCTs in dogs[9]. Response rates for vinblastine as a single agent in gross MCT disease have varied from 27% to 75%, but combination therapies including prednisone have shown improved efficacy[1][4]. ## Side Effects The predominant side effects of this chemotherapy combination are gastrointestinal in origin[1]. Hematological toxicity can also occur, with potential impacts on neutrophil count, hematocrit, reticulocyte count, and platelet count[3]. Vinblastine should be administered through an IV catheter to prevent potential injection outside the vein, which can cause severe tissue irritation[5]. ## Drug Interactions The risk or severity of adverse effects can be increased when vinblastine is combined with prednisone[6]. Additionally, grapefruit products should be avoided as they inhibit CYP3A metabolism, which may increase the serum concentration of vinblastine. Caution should also be exercised with St. John's Wort, which induces CYP3A metabolism and may reduce serum levels of vinblastine[6]. Veterinary oncologists typically aim to give doses low enough to keep side effects extremely low or nonexistent, balancing efficacy with quality of life[5].
