Overview

This is a combination therapy used primarily for hematopoietic stem cell mobilization in patients undergoing autologous stem cell transplantation. The triple combination works through complementary mechanisms: 1. **Vinorelbine** is a vinca alkaloid that acts as a microtubule destabilizing agent. It inhibits microtubule dynamics by binding to β-tubulin, preventing microtubule assembly and causing mitotic arrest[6][7]. When used for stem cell mobilization, vinorelbine serves as a chemomobilization agent. 2. **G-CSF** (Granulocyte-Colony Stimulating Factor) stimulates the bone marrow to produce and release hematopoietic stem cells into the peripheral blood circulation. The peak mobilization response to G-CSF alone typically occurs on the fifth day of administration[2]. 3. **Plerixafor** (also known by brand name Mozobil) is a CXCR4 antagonist that reversibly inhibits binding of stromal cell-derived factor 1 alpha (SDF-1α) to the CXCR4 receptor. This disrupts the retention of stem cells in the bone marrow, enhancing their mobilization into peripheral blood[2][5]. The triple combination has been shown to be superior in mobilizing peripheral stem and progenitor cells compared to double combinations (vinorelbine/G-CSF, G-CSF/plerixafor, or vinorelbine/plerixafor)[1]. This approach is particularly valuable for patients who are poor mobilizers or have failed previous mobilization attempts.
