Overview

This is a combination of three agents—vitamin B12 (cobalamin), folic acid, and nitrous oxide. Vitamin B12 and folic acid are essential water-soluble vitamins involved in DNA synthesis, red blood cell formation, and neurological function. Nitrous oxide is an inhaled anesthetic gas with analgesic properties but also acts as a potent inhibitor of vitamin B12-dependent enzymes. Nitrous oxide oxidizes the cobalt ion in vitamin B12, rendering it inactive; this disrupts methionine synthase activity, leading to impaired methylation reactions critical for myelin maintenance and DNA synthesis[1][3][4]. The resulting functional deficiency can cause megaloblastic anemia and subacute combined degeneration of the spinal cord even if serum B12 levels appear normal[3][4][10]. Folic acid supplementation may be used alongside vitamin B12 to support one-carbon metabolism but does not prevent neurotoxicity from nitrous oxide exposure if functional B12 deficiency persists[3][4]. This combination is not a standard pharmaceutical product or supplement; rather, it represents the interaction between these substances when administered together or encountered concurrently.
