Vitamin C + hydrocortisone is a combination therapy consisting of ascorbic acid (vitamin C) and the corticosteroid hydrocortisone. This combination has been investigated primarily for the treatment of sepsis and septic shock, often as part of a regimen that may also include thiamine (sometimes referred to as "HAT" therapy). Vitamin C acts as an antioxidant and supports immune function, while hydrocortisone is a synthetic glucocorticoid with potent anti-inflammatory effects. The rationale for combining these agents is based on their potential synergistic effects in modulating the dysregulated immune response seen in sepsis, reducing oxidative stress, and improving vascular function. However, clinical evidence regarding efficacy remains mixed; some early studies suggested reduced mortality and organ dysfunction with this regimen[1][2][3], but subsequent larger randomized trials have not demonstrated significant benefit over standard care[4][5][6]. The mechanism involves vitamin C's antioxidant properties and support of endothelial function, alongside hydrocortisone's suppression of proinflammatory gene transcription.
