Voralizumab (formerly SAN-300 and BIIB023) is a humanized monoclonal antibody designed to target the alpha-1 subunit of the Very Late Antigen-1 (VLA-1) integrin, also known as alpha-1 beta-1 (α1β1) integrin. VLA-1 is a collagen-binding receptor primarily expressed on activated T cells and other inflammatory cells, facilitating their recruitment and retention within inflamed tissues such as the synovium in rheumatoid arthritis. By binding to the I-domain of the alpha-1 subunit, voralizumab inhibits the interaction between VLA-1 and its ligands (collagen types I and IV), thereby reducing the inflammatory response. The drug was originally developed by Biogen Idec before being licensed to Valeant Pharmaceuticals (now Bausch Health) in 2012. It reached Phase 2 clinical development for the treatment of active rheumatoid arthritis in patients with an inadequate response to disease-modifying anti-rheumatic drugs (DMARDs), but development appears to have been discontinued.
