Vorasidenib plus pembrolizumab is an investigational combination therapy being developed by Servier for the treatment of IDH-mutant gliomas. Vorasidenib is an orally available, brain-penetrant small molecule inhibitor of mutant isocitrate dehydrogenase 1 and 2 (IDH1/2) enzymes. Pembrolizumab is a humanized monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor, acting as an immune checkpoint inhibitor to enhance T-cell anti-tumor activity. The rationale for the combination is to leverage the metabolic and epigenetic modulation of the tumor microenvironment by IDH inhibition to potentially sensitize the glioma to PD-1 blockade. It is currently being evaluated in a Phase 1 clinical trial (NCT05484622) for patients with recurrent or progressive IDH1-mutant astrocytomas and oligodendrogliomas.
