vorbipiprant + balstilimab

Vorbipiprant + balstilimab is an investigational combination drug regimen consisting of vorbipiprant, a potent and selective oral prostaglandin E2 receptor EP4 subtype antagonist, and balstilimab, a monoclonal antibody targeting programmed cell death protein 1 (PD-1). This combination is being developed to overcome resistance to immune checkpoint inhibition in tumors that are proficient in mismatch repair or microsatellite stable, particularly metastatic colorectal cancer (mCRC) and other gastrointestinal cancers. Vorbipiprant inhibits PGE2-EP4 signaling, which mediates tumor-associated immunosuppression, potentially sensitizing "cold" tumors to anti-PD-1 immunotherapy by reducing immune escape and enhancing immune responses. Balstilimab blocks PD-1, thereby preventing downregulation of immune responses by tumor cells. Clinical trials in heavily pretreated pMMR/MSS mCRC and metastatic GI cancers have indicated acceptable safety and promising activity, with disease control rates around 50% in mCRC and signals of efficacy in non-colorectal GI malignancies[1][3][7].
