Overview

Vorinostat (suberoylanilide hydroxamic acid) is a small molecule histone deacetylase (HDAC) inhibitor that targets Class I and Class II HDACs to modulate gene expression, induce cell cycle arrest, and promote apoptosis. In a clinical research program led by New York Medical College, vorinostat is being evaluated in combination with the VIT chemotherapy backbone—comprising vincristine, irinotecan, and temozolomide—for the treatment of pediatric and young adult patients with relapsed or refractory solid tumors and central nervous system (CNS) malignancies. This Phase I study (NCT04308330) aims to determine the maximum tolerated dose of vorinostat when added to the VIT regimen. The combination therapy targets multiple oncogenic pathways: vorinostat acts as an epigenetic modulator, vincristine inhibits microtubule formation, irinotecan inhibits topoisomerase I, and temozolomide serves as a DNA alkylating agent. The regimen is specifically being tested against a variety of childhood cancers, including Ewing sarcoma, rhabdomyosarcoma, neuroblastoma, Wilms tumor, and hepatoblastoma.
