Vortosiran (RBD4059) is a liver-targeted small interfering RNA (siRNA) therapeutic developed by Ribocure Pharmaceuticals for the prevention of thromboembolic disorders. It functions by silencing the expression of Coagulation Factor XI (FXI) in the liver, leading to a sustained reduction in circulating FXI levels. Factor XI is a key component of the intrinsic coagulation pathway, and its inhibition is hypothesized to provide effective antithrombotic protection with a significantly lower risk of bleeding compared to traditional anticoagulants like Factor Xa inhibitors or Vitamin K antagonists. Vortosiran is currently being evaluated in clinical trials for patients with non-valvular atrial fibrillation to assess its safety, pharmacodynamics, and bleeding profile relative to standard-of-care treatments like apixaban.
