VPI-2690B is a first-in-class, subcutaneously administered monoclonal antibody developed for the treatment of diabetic nephropathy and other diabetes-associated complications. It specifically binds to the C-loop domain sequence within the αVβ3 integrin receptor on kidney cells (podocytes, endothelial, and mesangial cells), which plays a role in the insulin-like growth factor-I (IGF-I) signaling pathway. In diabetes, high glucose levels lead to abnormal activation of αVβ3 by increasing its ligands, resulting in cellular changes that damage kidney filtration. By preventing ligand binding to αVβ3, VPI-2690B normalizes signaling and prevents or reverses further kidney damage. Preclinical studies have shown significant improvements in markers of kidney injury in animal models. The drug has completed Phase 1 trials demonstrating safety and is currently undergoing Phase 2 clinical trials for diabetic nephropathy[1][2][3][4][7].
