Overview

VV1 + cemiplimab is an investigational combination therapy being evaluated in the neoadjuvant setting for early-stage and locally advanced breast cancer, specifically targeting HER2-negative subtypes. **VV1** is an intratumoral oncolytic vesicular stomatitis virus (VSV) designed to induce immunogenic cell death and prime the tumor microenvironment by stimulating an innate and adaptive immune response. **Cemiplimab** (brand name Libtayo) is a fully human monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor on T-cells. By blocking the interaction between PD-1 and its ligands (PD-L1 and PD-L2), cemiplimab prevents the inhibition of T-cell activity, thereby enhancing the immune system's ability to attack cancer cells. This combination is part of the **I-SPY 2.2** trial, a phase 2 adaptive platform trial sponsored by **QuantumLeap Healthcare Collaborative**, which explores whether priming the immune system with an oncolytic virus can improve the efficacy of checkpoint inhibitors in immune-cold or immune-negative breast cancer tumors.
