Overview

A quadruple combination therapy comprising **VX-222** (a non-nucleoside inhibitor of hepatitis C virus [HCV] NS5B RNA-dependent RNA polymerase), **telaprevir** (a peptidomimetic inhibitor of HCV NS3/4A protease), **peginterferon-alfa-2a** (a pegylated recombinant interferon with antiviral and immunomodulatory activity), and **ribavirin** (a guanosine analog with broad-spectrum antiviral activity). This regimen was studied for genotype 1 chronic hepatitis C virus infection, with the primary indication being treatment-naive HCV genotype 1 patients. The combination was assessed for its efficacy in achieving a sustained virologic response (SVR), improved viral clearance, and safety/tolerability relative to standard-of-care regimens. The quadruple combination was explored in the phase 2 ZENITH study, with evidence for higher or comparable SVR rates compared to telaprevir-based triple therapy, but with increased risk of adverse events, especially gastrointestinal at higher VX-222 doses[1][3][5][7]. VX-222 and telaprevir both directly inhibit HCV replication by distinct mechanisms, while peginterferon-alfa-2a and ribavirin exert immunomodulatory and broad antiviral effects[1][5].
