Overview

Washed leukoreduced irradiated blood cell transfusions represent a specialized biological product used primarily in high-risk clinical settings, such as pediatric cardiac surgery. The preparation involves three distinct processing steps: **washing**, which replaces donor plasma with saline to remove inflammatory mediators (e.g., IL-6, IL-10), extracellular potassium, and plasma proteins that cause allergic reactions; **leukoreduction**, which removes donor white blood cells to prevent febrile non-hemolytic transfusion reactions and HLA alloimmunization; and **irradiation**, which uses ionizing radiation to inactivate donor T-lymphocytes, thereby preventing Transfusion-Associated Graft-Versus-Host Disease (TA-GVHD). This combination is specifically investigated for its ability to mitigate the systemic inflammatory response syndrome (SIRS) associated with cardiopulmonary bypass in children undergoing repair of congenital heart defects.
