The combination of whole blood, plasma, and packed red blood cells (PRBCs) represents a multi-component blood product resuscitation strategy used in the management of severe hemorrhage and trauma-induced coagulopathy (TIC). This combination is utilized as the standard of care (SOC) resuscitation protocol in the REPlaCE clinical trial (NCT04534751), led by Dr. Bellal A. Joseph at the University of Arizona. The protocol aims to address the lethal triad of trauma (coagulopathy, acidosis, and hypothermia) by restoring oxygen-carrying capacity through packed red blood cells and replenishing depleted clotting factors and volume through plasma and whole blood. In the context of the trial, this regimen is compared against the same protocol supplemented with 4-factor prothrombin complex concentrate (4-PCC) to determine if earlier factor replacement improves 30-day survival in patients with traumatic coagulopathy.
