Willfact is a high-purity, human plasma-derived von Willebrand factor (vWF) concentrate containing minimal amounts of Factor VIII. It is primarily indicated for the treatment and prevention of hemorrhage or surgical bleeding in patients with hereditary von Willebrand disease (VWD) when desmopressin (DDAVP) treatment is ineffective or contraindicated. In a Phase 2 clinical trial (NCT04110431) sponsored by Tirol Kiniken GmbH, Willfact is being investigated for its efficacy in treating acquired von Willebrand disease (aVWD) in adult patients undergoing extracorporeal membrane oxygenation (ECMO). During ECMO, high shear stress leads to the mechanical degradation of high molecular weight vWF multimers, resulting in aVWD and a significantly increased risk of bleeding. Willfact administration aims to restore these essential multimers, thereby improving hemostatic function and reducing the requirement for allogeneic blood product transfusions, such as packed red blood cells.
