This multimodal perineal analgesia protocol (MPAP) is a combination therapy being evaluated by the University of Miami for the management of postoperative perineal pain following vaginal surgeries such as perineorrhaphy or posterior colporrhaphy. The protocol consists of three components: medicated witch hazel pads (e.g., MAJOR Medi-Pads), which act as an astringent to reduce inflammation and swelling; a 20% benzocaine spray (e.g., Dermoplast), a local anesthetic that provides temporary pain relief by blocking sodium channels; and perineal ice packs for cryotherapy. This combination aims to provide a non-opioid alternative for managing localized pain and discomfort in the immediate postoperative period.
