WST04 is an orally administered live biotherapeutic product (LBP) developed by Shanghai Sine Pharmaceutical Laboratories for the treatment of advanced solid tumors. It consists of a genetically modified strain of *Bifidobacterium longum* that has been engineered to express and secrete an anti-Programmed Death-Ligand 1 (PD-L1) nanobody. This innovative approach utilizes the bacterium as a localized delivery vehicle, aiming to concentrate the checkpoint inhibitor within the tumor microenvironment or gut-associated lymphoid tissues. By blocking the PD-1/PD-L1 inhibitory pathway, WST04 seeks to enhance the activation and infiltration of tumor-specific T cells while potentially minimizing the systemic toxicities often associated with intravenous checkpoint inhibitors. The drug is currently undergoing Phase 1 clinical evaluation in China.
