**Wuse-oil Compound 03** is an externally applied oily traditional Chinese herbal medicine formulation used on the oral mucosa for radiation-induced oral mucositis. It has been used at China-Japan Friendship Hospital and was evaluated in a multicenter nonrandomized controlled clinical study in patients with grade 2 or higher radiotherapy-induced oral mucositis. Preclinical evidence indicates that the formulation modulates oral microbiota, reduces lipopolysaccharide-associated inflammatory signaling, suppresses Toll-like receptor 4 and nuclear factor kappa B pathway activity, and inhibits pro-inflammatory M1 macrophage polarization. Emodin was identified as a constituent with direct Toll-like receptor 4 binding activity in mechanistic experiments.
