Overview

Xalnesiran (RO7445482, RG6346) is a GalNAc-conjugated small interfering RNA (siRNA) therapeutic developed by Roche (following its acquisition of Dicerna Pharmaceuticals) for the treatment of chronic hepatitis B virus (HBV) infection. The drug utilizes the RNA interference (RNAi) pathway to target a highly conserved S region of the HBV genome, effectively silencing all HBV transcripts (including mRNA and pre-genomic RNA). This action leads to a profound and durable reduction in the production of viral proteins, most notably the hepatitis B surface antigen (HBsAg), which is a key factor in viral persistence and immune evasion. Xalnesiran is currently being investigated in Phase 2 clinical trials, such as the Piranga platform study, where it is evaluated as a monotherapy and in combination with other agents like TLR7 agonists or pegylated interferon to achieve a functional cure.
