**Xcel-m-condro-alpha** is an autologous advanced therapy medicinal product (ATMP) consisting of ex vivo expanded bone marrow-derived mesenchymal stem cells (MSCs). Developed by the **Banc de Sang i Teixits** (specifically its Xcelia division) and the **Institut de Teràpia Regenerativa Tissular (ITRT)**, it is primarily investigated for the treatment of knee osteoarthritis (gonarthrosis), targeting patients with Kellgren-Lawrence grade II to IV disease. The therapeutic process involves the harvesting of a patient's own bone marrow, from which MSCs are isolated and expanded under Good Manufacturing Practice (GMP) conditions over a period of approximately 21 days to achieve a therapeutic dose of roughly 40 million cells. The product is then administered via a single intra-articular injection. Xcel-m-condro-alpha works by modulating the intra-articular inflammatory environment through paracrine signaling (the secretome) and potentially through chondrogenic differentiation, which facilitates the repair and regeneration of articular cartilage. Clinical trials have demonstrated the therapy's safety and efficacy in significantly reducing pain and improving joint function, with MRI evidence supporting structural cartilage regeneration.
